- Print out the form and either turn it into FSR (located in 105 Kerckhoff Hall) no later than Monday, September 23 at the Recruitment Orientation or e-mail the form to FSR@ucla.edu.

- In order to participate in Panhellenic Recruitment, the cost is a NON-REFUNDABLE FEE of $50 before September 1st. The fee will increase to $60 after September 1st. After September 22nd, the fee will increase to $75.
- This fee includes all required Recruitment materials and will not be refunded if the Potential New Member withdraws from the Recruitment process for any reason.
